If you need your clinical records released, either through verbal communication or written documentation, please fill out an Authorization to Release form for each person or agency that you would like to receive the information.  If you are requesting your own written records (or those of a minor child), you still need to fill out a form and release the records to yourself.  Please note: to access forms, you will need to sign into the secure client portal.
